If you are an Accident or Healthcare customer and you need to make a claim, please notify us as quickly as possible, by calling us on (01 2696522) between 9am – 5pm Monday to Friday. We can provide further advice over the phone and post out anything you may need to help you make your claim.
If you are a Combined Benefit Services customer and need to make a claim, please click on the appropriate form below:
IMPORTANT: Once downloaded, the relevant form should be completed in full by you and your doctor and sent to us with your signature. On receipt of the completed form, we will review and assess your claim and policy coverage. We will then contact you confirming if/what benefits are payable. Please only use a continuation form if you need to continue an existing claim, otherwise use the new claim form indicated above.
